The Architecture of Aging: Why My Assessment Always Starts from the 'Outside-In'

In my 23 years of clinical practice, having performed over 40,000 treatments and authored 12 peer-reviewed journal articles, I’ve learned one immutable truth: the face is not a flat canvas. It is a complex, three-dimensional architectural masterpiece composed of distinct biological layers that age at different rates.

When patients come to see me at Artistrie Medical Aesthetics, they often point to a specific line, a nasolabial fold or a "marionette" line, and ask me to "fill it." But if I simply filled that line, I would be doing you a clinical disservice. I would be treating the symptom, not the cause.

The best practitioners know that to achieve a truly natural, "undone" look, we must respect the Architecture of the Face. This means moving away from "filling lines" and moving toward structural restoration. It also means having the clinical integrity to say "no" when a treatment isn't in your best interest.

The 4 Layers of Aging: Why We Look the Way We Do

To understand my methodology, you first have to understand what is happening beneath your skin. We don’t just age on the surface; we age from the bone up. Scientific literature, such as the landmark review "The Facial Aging Process From the ‘Inside Out’" published in the Aesthetic Surgery Journal, confirms that facial aging is a multi-layer phenomenon involving the skeleton, fat compartments, muscles, and finally, the skin.

1. Bony Deficits (The Foundation)

As we age, our bones actually recede. The "apertures" of our face: like our eye sockets: widen, and our jawline loses its projection. This is the loss of the "scaffolding" that holds everything else up. Without addressing the foundation, any work done on the surface will eventually sag.

2. Volume Loss (The Insulation)

We have deep and superficial fat pads that act as the face's "insulation." Over time, these fat pads atrophy (shrink) and shift. This is why we see hollowing under the eyes or the appearance of jowls. In my practice, I often use Sculptra Biostimulators to address this, as they work deep within the skin to stimulate natural collagen production, effectively "re-inflating" these areas naturally over time.

3. Muscle Movement (The Dynamics)

4. Skin Quality (The Exterior)

My Methodology: Top-to-Bottom, Outside-In

When I assess a face, I don't start with the wrinkle you're worried about. I follow a strict clinical protocol: Top-to-Bottom and Outside-In.

Why Top-to-Bottom?

Gravity is a constant. If we don’t address the forehead and temples (the "top"), we cannot expect the mid-face and jawline to look lifted. By restoring volume in the upper thirds of the face, we provide the "hoist" that the lower face requires.

Why Outside-In?

Imagine a tent. If the tent poles are sagging, you don't try to fix the canvas in the middle; you tighten the poles at the edges. The same applies to the face. We work from the periphery: the temples, the pre-auricular area, and the jawline: moving inwards. This creates a natural "pull" that smoothes the center of the face without making you look "done" or artificial.

The Golden Rule: LIPS GO LAST

If there is one phrase my students on Patreon hear me say more than any other, it is this: LIPS GO LAST.

Think of facial restoration like building and decorating a house. You wouldn't pick out the silk curtains (the lips) before you've laid the foundation (bone), built the walls (volume), and installed the wiring (muscles). If you put large, voluminous lips on a face that has lost its mid-face support and jawline definition, the result is the dreaded "duck-like" appearance that we actively avoid at Artistrie.

We build the house first. We ensure the architecture is sound. Only then do we add the finishing touches.

The "NO" Factor and Radical Honesty

In this industry, it is easy to find someone who will take your money for any treatment you request. I am not that practitioner. My background as an Adjunct Lecturer and a researcher with 6 "Best Paper" awards means I am tethered to evidence, not trends.

I am often asked about the latest social media fads: like "snatched" jawlines or certain trendy boosters. If the peer-reviewed data isn't there to support a treatment's safety or efficacy, I won't do it. I may choose not to treat you.

Radical honesty is the cornerstone of my practice. If I believe a treatment won't benefit you, or if I believe we are approaching a point of "over-correction," I will tell you. My goal is a refreshed, "restored" version of yourself: not a caricature.
